We are committed to your health and safety. Follow these tips to check in for your appointment:
- When you make your appointment, you will be asked to print out and complete the appropriate forms (link to website forms); for example:
- Patient Demographics
- Confidential Channel Communication Request
- Health Visit Questionnaire
- Well Check/Annual Visit
- Request or Patient Access to Health Information
- When you arrive at the office building
- If you prefer to stay in your car, please call the office upon your arrival.
- When you enter the building, your temperature will be taken in the lobby.
- All people who enter the building must wear a mask.
- Only patients are allowed in the office. No friend or family members can accompany the patient.
- If you are not feeling well or if your temperature is 100.4 or greater, your appointment will be cancelled, and we ask that you contact your primary care doctor. Your appointment will be rescheduled.
- When you arrive at our office, turn in your completed forms.
